Important Life Insurance Details:

No Life coverage change goes into effect until you and EBR Schools are notified by Sun Life.

Optional life terms at retirement.

The Sun Life Policy Dependent Optional Life Insurance contains restrictions on the amount of coverage that is allowed by the Sun Life Policy. The amount of Dependent Optional Life Insurance can not exceed 50% of an Employee or Retiree's amount of Basic and Optional Life Insurance combined. The amount after calculated the 50% amount is rounded down to the amount of an available option. For example, an Employee who was hired after July 1, 2002 whose Basic Maximum Benefit is $50,000 and the Employee did not elect any Optional Life Insurance, the Dependent Optional Life Insurance is limited to an Option that will provide coverage for the Spouse as Option IV - $20,000/$4,000, Option V - $10,000/$2,000 or Option VI - $5,000/$2,000. (50% of $50,000 is $25,000 which must be rounded down to an available Option).

Another example is an Employee whose Annual Salary is $9,000 and is allowed a Basic Maximum Benefit of 1 ½ times the Employee's Annual Salary or in this example, $13,500. As a result, the amount of Dependent Optional Life Insurance that is available is limited to Option VI, since the Optional Life Insurance can not exceed 50% of the Employee's basic Life Insurance coverage, which in this case is $6,750 which amount is rounded down to the available Option, namely Option VI - $5,000/$2,000.

An Employee, who was hired after July 1, 2002 and at the time of employment selected Optional Life Insurance, may be eligible for an amount of Dependent Optional Life Insurance in a greater amount than stated above in the example. However, in applying the 50% limitation, it is necessary to round down to the next available Dependent Option.

No changes will be made regarding any requested changes involving the Sun Life Insurance until, and unless Sun Life has received and approved the Evidence of Insurability. An Evidence of Insurability is required to be submitted in the case of Dependent Optional Life Insurance for the Spouse, as well as for each Child.
